Parental Waiver of Liability and Hold Harmless Agreement
Minor 18 Years & Younger

1. I understand that Mundi Limousine LLC (“MUNDI”) will be providing transportation for minors on ___DATE______, and that only parents or legal guardians are permitted to make reservations for minors, including young adults 18 years and under if the reservation is for a school related function or if minors 17 years and younger will be included as a passenger in the reservation. Parental permission is required on all transportation without adult supervision for minors 18 years and under and for minors 16 years and under with adult supervision.

    

2. I acknowledge that MUNDI and it’s chauffeur assigned to my child’s service are not chaperones and will not be responsible or held liable for the decisions or behavior of my child.

    

3. In consideration for receiving permission for my child to participate with MUNDI, I hereby RELEASE, WAIVE, DISCHARGE AND HOLD HARMLESS, MUNDI and their officers, agents, employees, and chauffeurs, from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child while utilizing the transportation service.

    

4. I am fully aware of the risks associated and hazards connected with this activity, including but not limited to travel risks, inclement weather, and/or road hazards. I hereby elect to consent and allow my child to voluntarily participate in said activity with full knowledge that said activity may be hazardous to my child. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH that may be sustained by my child, as a result of engaging in such activity.

    

5. I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS MUNDI, their officers, agents, employees, and chauffeurs, from any loss, liability, damage or costs, including court costs and attorney fees, that they may incur due to my child’s participation in said activity.

    

6. I further acknowledge the following rules governing my child’s transportation and agree to the following additional terms and conditions:

a. No alcohol, drugs, cigarettes or other intoxicants are allowed in the vehicle.

b. In the event chauffeur finds drugs or alcohol, the service will be terminated immediately, parents will be contacted and no refunds shall be issued; Driver retains authority to return to the point of pickup or end the service at the point of infraction.

c. All packages must be inspected by the chauffeur and will remain in the trunk of the vehicle.

d. The privacy screen remains open at all times.

e. Passengers must remain seated at all times while vehicle is operating.

f. Passengers are prohibited from throwing items in vehicle.

g. Each parent of a minor passenger must provide a phone number and contact in case of a violation or emergency.

h. Only minors registered below as a passengers are permitted transportation in the vehicle.

i. Signatures beside each name indicates this contract is accepted by parents and passengers listed and that a representative from Mundi Limos has the permission and right to inspect all packages accordingly.

j. Parent’s agree to reimburse Mundi Limos for any damages caused by any minor during the rental agreement whether negligent or accidental, which will be billed to the parent’s credit card on file, or the guaranteeing credit card, upon notification of damages.

k. If a minor requires immediate medical attention during service, parent authorizes MUNDI to seek reasonable and necessary medical treatment for minor and agrees to be responsible for any such cost incurred as a result of such treatment.

7. IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made; I further attest that I am the parent or legal guardian of the minor child who’s name is indicated below, that I at least eighteen (18) years of age and fully competent; and I execute this Release for full, adequate and complete consideration fully intending to be bound by same.
